Why do I need to fill out my forms before each follow-up appointment again?

One of the most important aspects of patient care for Frida’s team is patient safety. We want to do our utmost to ensure we understand your situation and not only your medical history, but how thing are going for you in your life going forward. Things change. People change. Circumstances change.

By thoroughly assessing how you are doing and feeling with your new medications for each of your appointments, we can have a high degree of confidence that our recommended approach is suitable for what is going on for you in your life. And this best enables us to better support you.

We recognize that it can sometimes feel redundant to fill out the forms again. We really appreciate you doing so - as it’s an intentional and important part of assessing your on-going care.
